Your Home Care, LLC, Owner of is committed to respecting your privacy and protecting your personal information.  This Privacy Policy describes how we collect, use, share and protect your information.  By using our Site, or signing up for our services, you consent to our collection and use of your information as described in this Policy.


How We Collect Your Personal Information


We may collect personal information directly from you when you visit our Site, sign up for our services or voluntarily participate in our programs. Health and Clinical Data


Health Information that we may collect may include medical history, prescription and over-the-counter medications taken, clinical data such as diagnosis and treatment information, laboratory tests, patient insurance, physiologic monitoring data, hospitalization, administrative and demographic information.


Customer Records and Commercial Information


We may collect financial information, such as payment details, credit card numbers, bank or customer account information, transaction records.  In connection with our processing, legal compliance, defense, or pursuit of legal and/or insurance claims, this information may also include Social Security Number or other state, national, or tax identification numbers.



Professional or Employment Related Information and/or Education Information


We may collect information relating to your professional or employment history, education credentials and history or other similar information submitted by you in connection with employment, internment or scholarship application maintain by the company in connection with your employment at the company or provision of services to the company by you or your employer, or shared with prospective employers by you on professional listing services and professional social media services.


Geolocation Information


We may collect information such as site, time, and date of purchases or data derived from your IP address, country and zip code.


Inferences Drawn from Other Personal Information


This category may include inferences drawn from the above information that may reflect your preferences, characteristics, predispositions, behavior, attitudes, or similar behavioral information.  In certain limited cases such inferences may be drawn in connection with sensitive or potentially sensitive personal information you may provide to us.

  • Assessing Clients’ unique needs and developing a care plan of action

  • Coordinating and providing caregiving services

  • Matching caregivers to Clients

  • Managing payment and insurance claims

  • Preventing fraudulent activity

  • Meeting legal or regulatory requirements

  • Responding to your requests for information

  • Administering and providing other online service

  • Marketing and/or advertising

  • Background checked

Disclosure of Personal Information


If you provide your personal information to us and you use our services, we may disclose your personal information to the following:


  • Caregivers and other service providers who serve our Clients.

  • Insurance providers, government agencies, and other third-party service providers as necessary to render our services to you.

  • Legal and professional advisors and consultants.

  • Law enforcement agencies and other parties as required by law or to protect our rights and interests.




We maintain reasonable safeguards to protect the integrity and confidentiality of your personal information from unauthorized access, use, disclosure, alteration, or destruction.  We have complied with the HIPAA security rule for administrative, technical, and physical security safeguards and share information only on a “need to know” basis. However, please be aware that no security measures are perfect or impenetrable and we cannot guarantee the absolute security of your information. We will do our part to protect your information, but it is important for you to protect your information as well. Some of the steps we are taking include:

Your Rights

Right to delete personal information.  You have the right to request that we delete the personal information we have collected about you.


Right to correct inaccurate personal information.  You have the right to request that we correct and accurate personal information that we maintain about you.


Right not to be discriminated against for exercising the rights identified above.  You have the right not to be denied goods or services, charged different prices or rates for goods or services, or receive a differing level of quality of goods or services as a result of exercising the above rights.


Covered Personal Information


The rights described above do not apply to certain categories of personal information.  The personal information excluded from coverage under the CPRA includes, among other things:


  • Personal information we collect from or about you when you apply for or obtain a product or service for personal, family or household purposes; and

  • Personal information sold or bought from a consumer reporting agency for use in a consumer report in compliance with applicable law.


We will apply verifiable consumer requests to cover personal information.

In order to provide a response, we may ask that you provide clarifying or identifying information to verify your request.  We will respond to authorized requests within the timeframe required by law.  The above rights are subject to our ability to reasonably verify your identity and authority to make these requests.


You may designate an authorized agent to submit a request on your behalf by providing that agent with your written permission. If an agent makes a request on your behalf, we may still ask that you verify your identity directly with us before we can honor the request. 

Agents who make requests on behalf of individuals will be required to verify their authority to make the request by submitting written authorization from the individual.

Your California privacy rights are subject to various exclusions and exceptions under the law.  Under certain circumstances, we may be unable to implement your request, pursuant to applicable law.  We will advise you of any reason for denying or restricting a request to the extent permitted by law.


We will not provide social security numbers, driver’s license numbers or government issued identification numbers, financial account numbers, health insurance or medical identification numbers, account passwords or security questions and answers, or other specific pieces of personal information if the disclosure presents an unreasonable risk to the security of personal information, customer accounts or our systems and network security.
